Definition of Transition Doc

A document used when an employee leaves a company to ensure that their knowledge is shared with the team and any open tasks are reassigned. It should include the employee’s responsibilities, any tasks they have in progress, any knowledge they have that needs to be passed on, and any contacts they had that may need to be reassigned. It should also include any decisions that were made and any changes that were made to processes, procedures, or systems. This document is important in ensuring that the transition is as smooth as possible for both the employee and the team.
